Male infertility is often caused by a low sperm count or an anatomical abnormality, such as a palpable dilation of veins in the scrotal area known as varicocoele or small sized testicles. Other contributing factors can be attributed to how the sperm move (motility), or an abnormal sperm type. A few reasons for low sperm count include prolonged fever or a recent severe illness, excessive alcohol consumption, endocrine disorders, testicular injury and exposure to toxins, radiation or high heat.

The first step in infertility management is to undergo some infertility tests like [1]Transvaginal Scan [2] SIS [Saline Infusion Scan], [3] HSG [Hysterosalpingography] [4] Sono-Hydrotubation [5]Folliculometry [Serial ovulation scans], [6] Hormonal Profiles, [7] HVS, [8] Sperm test, [9] Scrotal Scan, etc.

Assisted Pregnancy Techniques  also called Assisted Reproductive Technology [ART] refers to a number of advanced techniques that aid fertilization and pregnancy. These techniques are often used for women who have irreversible damage to their fallopian tubes or cervical mucous problems, men with low sperm count. In this write up we will be particularly looking at the simplest and cheapest but still very effective assisted pregnancy techinique  called  Intra Uterine Insemination( IUI ).   Intrauterine insemination (IUI) is a procedure for treating infertility. IUI involves a laboratory procedure to separate fast moving sperm from more sluggish or non-moving sperm. The fast moving sperm are then placed into the woman’s womb close to the time of ovulation when the egg is released from the ovary in the middle of the monthly cycle. 

IUI is most commonly used for unexplained infertility. It is also used for couples affected by fibroids, mild endometriosis, problems with ovulation , male factor infertility  such as low sperm count and cervical factor infertility.

Intrauterine insemination is performed by threading a very thin flexible catheter through the cervix and injecting washed sperm directly into the uterus. The whole process doesn't take very long ,it usually only requires the insertion of a speculum and then the catheter, a process that may take a couple of minutes (60-90 seconds to introduce the catheter, then sperm injection, and another 60 seconds or so to remove the catheter, going slowly helps reduce discomfort). Sometimes when the cervix is hard to reach a tenaculum is used to hold the cervix, which makes the process a bit more uncomfortable.

I must say I agree with you completely, there is definitely no clear relationship. The topic came to me, through a funny experience I had last week at the clinic. Uterine fibroids also known as myomas, leiomyomas or fibromyomas are non-cancourous tumor growths of the uterine. These often appear during child bearing years of women and are rarely associated with an increased risk for cancer. Uterine fibroids can show no signs and symptoms for most of the women, however most common symptoms include heavy menstrual bleeding, pelvic pain or pressure, constipation, bleeding between periods or prolonged menstrual periods, frequent urination, leg pain and backache.

Normally, it is believed that fibroid tumors do not have any effect on fertility and pregnancy, but studies have revealed that in about 3% of women, infertility is caused by uterine fibroids. Mainly large, multiple and pedunculated or stalked uterine fibroids are the real culprits.

Enlarged subserosal uterine fibroids may induce infertility in two ways:

[1] By compressing the fallopian tubes and blocking the passage of sperm and egg.

[2] By distorting the pelvic anatomy to such an extent that it becomes difficult for the fallopian tube to capture an egg at the time of ovulation.

Large or multiple intramural and submucosal fibroids are also not far behind when it comes to inducing infertility. These tumors of the uterus can cause infertility in a myriad of ways.

[1] They may block the fallopian tube, thereby preventing sperm from fertilizing the egg.

[2] They may increase the size of the uterine cavity, which increases the distance that sperm have to travel.

[3] Both intramural and submucosal fibroids may have a severe impact on the uterus’ ability to contract, which in turn can interfere with sperm migration and ovum transport.

[4] Multiple and large submucosal fibroid as well as intramural fibroid tumors can distort the entire anatomy of the uterus. In such a situation, even if the sperm is able to fertilize the egg, the chances of that egg implanting are drastically reduced.

From the write up about fibroids, you can see that it can affect most women getting pregnant, but with sperm wash and IUI, where we will inject the sperms very close to the fallopian tubes, we can shorten the distance the sperm will travel, because fibroids really expands the womb.  The first thing to do is to undergo some infertility tests, once we see that all is clear, we can go ahead and try assisted pregnancy techniques, keep our fingers crossed and believe God, the next pregnancy may just be yours.

Infertility is the inability of couples to get pregnant after one year of frequent unprotected sex, if the lady is 30 years and above, we reduce it to six months.  There are many causes of infertility, from tubal factor to ovarian and male factor, I am not writing about that today, however I have many articles at joasmedical.com. After many couples have tried for pregnancy and are not getting anywhere, I am now encouraging them to be adventurous and try assisted pregnancy techniques. Assistant pregnancy techniques like intrauterine insemination are getting very popular nowadays because it is simple natural and relatively cheap. Yes I said relatively cheap, because you will need to spend some little extra amount of money for this technique, one thing I have noticed from many clients is that they want to get pregnant but are not willing to spend any money, I have seen many clients put on some relatively expensive clothing that are even more than N100000 each time they visit the clinic, but each time you talk about spending a little extra money to test or treat their infertility, they recoil and prefer to stay infertile. The truth is that as far as there is no health insurance, you have to be personally responsible for the cost of treatment of your infertility.  So we should not be shy when it comes to spending a little extra money for assisted pregnancy techniques, it may just be the only thing you need to overcome your infertility and get as many pregnancies as you wish.

Now, where does sperm wash come into this discussion, yes, it is very important when you talk of assisted pregnancy techniques , because we must only use the best sperm cells to be able to get the best results.  Sperm wash is especially suited for men with low sperm count and also for couples with unexplained infertility [ this is where every thing is okay with the couple but they just cannot get pregnant] ,  we use this method to remove the dead or poorly motile sperm and use only the few best to inseminate the lady. Sperm wash is a procedure done to separate individual sperms from the seminal fluid, this is done so that the healthiest sperms are used for assisted pregnancy treatments.  Raw sperms cannot be inserted directly into the woman’s womb, because raw sperms contain a chemical substance called prostalglandis, which cause muscular contractions and are also responsible for cramps during menstruation and pregnancy. Sperm wash remove dead and poorly motile sperms, it also gets rid of white blood cells, mucous and seminal fluid, that can negatively affect fertility.

Sperm wash is a special laboratory procedure that require the use of some few equipment like centrifuge, microscope and incubator, also we use various types of expensive reagents. There are different methods, but the more frequently used methods are Direct Swim Up, Swim Up and Density Gradient. The most popular of these methods is the density gradient swim down , because most cases of assisted pregnancy techniques are for clients with low sperm count, this particular method is known to do a thorough job of washing the sperm and giving us the few best cells for insemination.

Some of you may be thinking of choosing the sex of your baby, during sperm wash, we can separate the male and female sperm cells, using a special technique, and this method is known to be 70% accurate for determining the sex of the baby, however it is more expensive.  So  you can see that there is no need to continue to tolerate and manage your infertility state, take action, be adventurous, try something new and  maybe it will just happen. Her case is just one, there has been hundreds before we launched the system online last year. For over 12 years we have been using the system and has been getting results. It has been found by research that in most cases of low sperm count, treatment regimes  have helped but have not really improve the sperm counts more than one or two million, so if the man sperm count is 20 million and below, we will not encourage them to continue a life long fruitless therapy, but go ahead and have a Sperm Wash with assisted pregnancy techniques.

Another group of clients that have had poor success with the secret fertility system are those ladies with fibroids , polycystic ovaries, severe  menstruation and ovulation problems.  This year they will all be encouraged to try a series of Assisted Pregnancy Techniques, at least 3 to 6 series, which we believe  will be able to help them achieve pregnancy.

I am sure you have been asking this question "where is Dr. Victor?" Your concerns are understood. Other friends on my list have also been asking the same questions. Your imaginations are not far from the truth. I have actually been travelling for some weeks, I was in Dubai for a medical conference called Arab Health 2011, and immediately moved to Chennai in India to attend a very important short course on infertility , intrauterine insemination and IVF. Do not panic , by God's grace I will be back shortly.
This is actually my first time to travel to India, and my stay so far have been very interesting, medical practice here is really very advanced, I now understand why many doctors and patients come here. I myself took out time to visit the dentist, the service was excellent and charges very cheap compared to Naija.



I could have written earlier but there was no opportunity to use the internet. Now I have access, so I have quickly put up a little write up on miscarriage. This is a very important topic, many clients that I know suffering from infertility have also claimed to have had one or many cases of miscarriages. Infact a lady that has not had a life baby and have always been having recurrent miscarriages for over one year is infertile and need medical attention.

Miscarriage is the sudden end of a pregnancy at a stage where the embryo or fetus is incapable of surviving ,
generally defined as prior to 20 weeks of gestation. You are likely to suffer a miscarriage if you are 28 years and above, and if you notice from my previous writings you would have noticed that I always emphasize that infertility is related to age. So if you are 38 years and above and have been seeking pregnancy for some years, please also try some other reproductive techniques such as intrauterine insemination .

There are some symptoms you can have if you are having a miscarriage, this include
[1] Vaginal bleeding [always]
[2] Pelvic pains or abdominal cramps, most likely with a latter bleed
[3] Passage of tissues, resembling large thick blood clots.

When you suspect that a miscarriage is about to occur, occuring or have already occured, one test is very important, that is an ultrasound scan. A scan can tell if the pregnancy is still intact and alive [ that is in cases of threatened miscarriage], it can tell if the cervix is opened, or in case it has already occured, it can tell if there is any retained products of conception that need to be removed to make the womb free for another pregnancy to occur.



There are many causes of miscarriage, this include
[1] Chromosomal abnormality, this is one of the major causes of miscarriages especially in the first trimester [ prior to 13 weeks gestation]. An abnormal sperm or egg will eventually form an abnormal pregnancy, that will eventually come down, so that no abnormal baby is born.
[2] Other causes are those of uterine abnormalities, growth in the uterus like fibroids or polyps, and also weak cervix.
[3] Hormonal problems
[4] Pelvic infections
[5] Immunological disorder- This is an immune disorder that can be due to blood incompatibility or sperm reaction, and can lead to miscarriages Knowing the causes of miscarriages will help us to prevent it or reduce their occurencies. One wonderful way of reducing miscarriages is to have sex without barrier contraceptives, especially oral sex. Your husband's semen [sperms] during sexual activities have been proven to decrease your chances of any immunological disorder that can occur during pregnancy, so decreasing miscarriages. Immunological tolerance is most quickly established through oral introduction and gastrointestinal absorption of semen [ that means pregnant ladies should give their husbands a great blow job to reduce their chances of having a miscarriage, and do not forget to swallow some of the highly proteineous and nourishing semen --- This will definitely make the men happy]
If you see any amount of blood from your vagina at any stage of your pregnancy, PLEASE, do not manage it at home at all, JUST RUN TO THE HOSPITAL AND SEE YOUR DOCTOR.

How is it possible for sex to cause me infertility?

After all it is suppose to be the cure of my infertility, not the cause.

Just imagine the darkness that could come into your world if sex is taken away .

Imagine all the fun you will miss

However I am sure many people will still accept it, so that they could at least have a baby or two

But in reality, can sex really cause infertility?

To me the answer is a capital NO

So if the answer is NO, that means the topic is just to catch your attention.

Not exactly the truth, because some minority school of thought really believe that sex can cause infertility.

Why this thinking?

It is believed that the sensation of sex, especially very rough sex can lead to some abnormal spike in hormones , eventually leading to hormonal imbalance, irregular menstruation and irregular ovulation leading to infertility.

One established fact is that too much and continuous sex can lead to low sperm count, because  with  continuous  sex , he releases sperm faster than the testicles can produce, this can lead to infertility.

The next few days will usher us into the Xmas and new year holiday

In that kind of mood, there is no place for boredom

It is a time of sex and more sex and more sex

I can assure you that, I am in total support of more sex at this time as a cure for infertility and especially as a source of enjoyment

Two points about sex for a successful conception during this period are

Firstly, to not only have sex , but to have more passionate  sexual activities with foreplays, sweet talks and plenty love, this will release the right hormones that will be balance and help ovulation to occur leading to a successful conception.

Secondly, the best sexual position for a successful conception is one that places the penis directly and very close to the cervix, so that sperm can swim straight into the cervix after ejaculation. That is why the man on woman [ or missionary] position is the best for a successful conception.

Finally, if you have been married for 6 months and above, you must undergo some studies that are known to  improve fertility that will lead to a successful conception.

The major one is called Hysterosalpingography , popularly called HSG, this test is so good in that it is known to help clear the fallopian tubes of all debris and mucous so helping sperm cells to swim freely and reach the eggs easily.

Another is almost similar to HSG it is called Sono-Hydrotubation, especially good for those that have been diagnosed with tubal blockage.

Transvaginal  Folliculometry, which is a serial monitoring of ovulation, is also a powerful study that can tell us the size of the egg, type and day of ovulation.

With these studies and more , accompanied with some fervent and passionate sex this holiday, you will definitely be a winner.
