Now, where does sperm wash come into this discussion, yes, it is very important when you talk of assisted pregnancy techniques , because we must only use the best sperm cells to be able to get the best results. Sperm wash is especially suited for men with low sperm count and also for couples with unexplained infertility [ this is where every thing is okay with the couple but they just cannot get pregnant] , we use this method to remove the dead or poorly motile sperm and use only the few best to inseminate the lady. There are different methods, but the more frequently used methods are Direct Swim Up, Swim Up and Density Gradient. The most popular of these methods is the density gradient swim down , because most cases of assisted pregnancy techniques are for clients with low sperm count, this particular method is known to do a thorough job of washing the sperm and giving us the few best cells for insemination.

Some of you may be thinking of choosing the sex of your baby, during sperm wash, we can separate the male and female sperm cells, using a special technique, and this method is known to be 70% accurate for determining the sex of the baby, however it is more expensive. MISCARRIAGE EXPLAINED , A FORM OF INFERTILITY

Hi Friend,
I am sure you have been asking this question "where is Dr. Victor?" Your concerns are understood. Other friends on my list have also been asking the same questions. Your imaginations are not far from the truth. I have actually been travelling for some weeks, I was in Dubai for a medical conference called Arab Health 2011, and immediately moved to Chennai in India to attend a very important short course on infertility , intrauterine insemination and IVF. Do not panic , by God's grace I will be back shortly.
This is actually my first time to travel to India, and my stay so far have been very interesting, medical practice here is really very advanced, I now understand why many doctors and patients come here. I myself took out time to visit the dentist, the service was excellent and charges very cheap compared to Naija.

I could have written earlier but there was no opportunity to use the internet. Now I have access, so I have quickly put up a little write up on miscarriage. This is a very important topic, many clients that I know suffering from infertility have also claimed to have had one or many cases of miscarriages. Infact a lady that has not had a life baby and have always been having recurrent miscarriages for over one year is infertile and need medical attention.

Miscarriage is the sudden end of a pregnancy at a stage where the embryo or fetus is incapable of surviving ,
generally defined as prior to 20 weeks of gestation. You are likely to suffer a miscarriage if you are 28 years and above, and if you notice from my previous writings you would have noticed that I always emphasize that infertility is related to age. So if you are 38 years and above and have been seeking pregnancy for some years, please also try some other reproductive techniques such as intrauterine insemination .

There are some symptoms you can have if you are having a miscarriage, this include
[1] Vaginal bleeding [always]
[2] Pelvic pains or abdominal cramps, most likely with a latter bleed
[3] Passage of tissues, resembling large thick blood clots.

When you suspect that a miscarriage is about to occur, occuring or have already occured, one test is very important, that is an ultrasound scan. A scan can tell if the pregnancy is still intact and alive [ that is in cases of threatened miscarriage], it can tell if the cervix is opened, or in case it has already occured, it can tell if there is any retained products of conception that need to be removed to make the womb free for another pregnancy to occur.

There are many causes of miscarriage, this include
[1] Chromosomal abnormality, this is one of the major causes of miscarriages especially in the first trimester [ prior to 13 weeks gestation]. An abnormal sperm or egg will eventually form an abnormal pregnancy, that will eventually come down, so that no abnormal baby is born.
[2] Other causes are those of uterine abnormalities, growth in the uterus like fibroids or polyps, and also weak cervix.
[3] Hormonal problems
[4] Pelvic infections
[5] Immunological disorder- This is an immune disorder that can be due to blood incompatibility or sperm reaction, and can lead to miscarriages Knowing the causes of miscarriages will help us to prevent it or reduce their occurencies. One wonderful way of reducing miscarriages is to have sex without barrier contraceptives, especially oral sex. Your husband's semen [sperms] during sexual activities have been proven to decrease your chances of any immunological disorder that can occur during pregnancy, so decreasing miscarriages. Immunological tolerance is most quickly established through oral introduction and gastrointestinal absorption of semen [ that means pregnant ladies should give their husbands a great blow job to reduce their chances of having a miscarriage, and do not forget to swallow some of the highly proteineous and nourishing semen --- This will definitely make the men happy]
If you see any amount of blood from your vagina at any stage of your pregnancy, PLEASE, do not manage it at home at all, JUST RUN TO THE HOSPITAL AND SEE YOUR DOCTOR.

How is it possible for sex to cause me infertility?

After all it is suppose to be the cure of my infertility, not the cause.

Just imagine the darkness that could come into your world if sex is taken away .

However I am sure many people will still accept it, so that they could at least have a baby or two

But in reality, can sex really cause infertility?

To me the answer is a capital NO

So if the answer is NO, that means the topic is just to catch your attention.

Not exactly the truth, because some minority school of thought really believe that sex can cause infertility.

Why this thinking?

It is believed that the sensation of sex, especially very rough sex can lead to some abnormal spike in hormones , eventually leading to hormonal imbalance, irregular menstruation and irregular ovulation leading to infertility.

One established fact is that too much and continuous sex can lead to low sperm count, because with continuous sex , he releases sperm faster than the testicles can produce, this can lead to infertility.

The next few days will usher us into the Xmas and new year holiday

In that kind of mood, there is no place for boredom

It is a time of sex and more sex and more sex

I can assure you that, I am in total support of more sex at this time as a cure for infertility and especially as a source of enjoyment

Two points about sex for a successful conception during this period are

Firstly, to not only have sex , but to have more passionate sexual activities with foreplays, sweet talks and plenty love, this will release the right hormones that will be balance and help ovulation to occur leading to a successful conception.

Secondly, the best sexual position for a successful conception is one that places the penis directly and very close to the cervix, so that sperm can swim straight into the cervix after ejaculation. That is why the man on woman [ or missionary] position is the best for a successful conception.

Finally, if you have been married for 6 months and above, you must undergo some studies that are known to improve fertility that will lead to a successful conception.

The major one is called Hysterosalpingography , popularly called HSG, this test is so good in that it is known to help clear the fallopian tubes of all debris and mucous so helping sperm cells to swim freely and reach the eggs easily.

Another is almost similar to HSG it is called Sono-Hydrotubation, especially good for those that have been diagnosed with tubal blockage.

Transvaginal Folliculometry, which is a serial monitoring of ovulation, is also a powerful study that can tell us the size of the egg, type and day of ovulation.

CAUSES OF INFERTILITY REVISITED FOR SUCCESSFUL PREGNANCY

Hi My Friend,

I will start this article by first making out a heart felt APOLOGY to you my friend that has been trying to see me personally at the clinic.

Especially for you that I booked an appointment to meet this last Saturday.

I am really very sorry that I could not make it.

I did all I could to be there, but somehow a few hours to that time I had a very important emergency issue that I must attend to.

So I had no other choice but to disappoint you.

I hope I can make it up to you some other time.

I can understand the stress of your coming all over the state to see me at Ikotun, especially going through all those wicked but wonderful traffic caused by the endless and yearless road constructions that seem to be ongoing for life, never having a time it will end.

I believe one day, they will eventually do the right thing and really construct the roads in Lagos, like they are doing successfully in Calabar [ where I recently visited].

I really appreciate you my friend that take out of your tight schedule to come to the clinic to see me, especially those of you coming from very far and out of the state.

I remember my friend and client from Kaduna, you too that came all the way from Ibafon and you also that came from Abeokuta. It was good that I was able to see all of you, may be it was because you came during the week. I believe that our discussion was very fruitful, and we should be seeing the end of infertility in your life soonest.

I know that weekends are especially busy and crazy times for me, but I will still make out time one or two Saturdays in a month to try and meet with you that is busy during the week.

Unfortunately this coming Saturday I will also not be at the clinic. A very close friend and brother of mine who is a medical doctor and also a solder travelled all the way from Darfur Sudan [Peace keeping force] to get married to his heart throb, and I must be there, since it is my wedding.

So there is no way I will be at the clinic this coming Saturday.

However I am not a holiday person , so those of you who will not be travelling out of Lagos, or those who will be coming into Lagos from outside the state during this Xmas season, and will like to see me personally, can easily see me at the clinic during this time, because I will be working full time on Xmas, boxing and newyear days.

But the greatest way to contact me and ask any question in your mind is to send me a simple email, I will surely reply you.

Do not also think I do not find time to relax, I do, I will surely be taking the kids out to either the beach or the Apapa Amusement park, one of the week days during the festival time.

You can also suggest some great fun places for me at Ikeja where I can take the kids to for fun.

I only know the Apapa Amusement park and also the many beaches in Lagos, I wish I can find a near by place at Ikeja to enjoy myself with the family.

Talking about the kids. My first daughter [my ADA] was 9 years old this Sunday. She had a low keyed birthday celebration, but it was still fun.

Wao, she is really growing into a beautiful and lovely lady. Seeing her grow make me imagine how much I will be collecting for her bride price. Maybe I will not take naira, I need some hard currency, so I will either take Dollars, Pounds, or Euros.

Infertility is really looking broad, and becoming a major issue worldwide. I have tried in my articles to you to make it as simple and user friendly as possible.

The reason is this, so that you will be so well informed that you can easily understand it and through that find solution to it.

Infertility is just the inability to get pregnant after one year of regular sexual intercourse. If you are 30years and above, then it will be after six months.

In this article we want to remind ourselves some of the causes of infertility, because as the saying goes, once you know the cause of a problem, you have gotten 60% of the problem solved.

The major cause of infertility remains ovulation disorder. This is very important, because if you have problems with your ovulation , you will not be able to produce eggs for your partner sperm to fertilise. Ovulation problem can be so serious, because it can after a lady at anytime, since it is heavily dependent on her hormone status. That is why you can see a lady that has kids and is now seeking another one , but finding it difficult.

Another cause of infertility that is affecting many ladies in Nigeria is Blocked Fallopian Tubes. Yes this is serious, a lady can produce eggs, and her husband’s sperms can be superb, but if there is any blockage in her fallopian tubes , there is no way the sperms can get to the eggs, so no pregnancy can occur.

Tubal blockage can be caused by a very minor reason like a thickened Mucous plug, this is the simplest form of partial or total blockage that can delay any lady at any age and anytime from getting pregnant. However the beauty of Mucous Plug tubal blockage is that it is easily treated by just a diagnostic test like HSG or Sonohydrotubation.

Some more serious causes of tubal blockage are PID. Endometriosis, ectopic pregnancy leading to the tube been surgically removed, other surgeries from ruptured appendicitis, even complications from C&S and abortions.

The next is uterine scarring from previous abortions, this is serious, and can really be long standing.

Do not forget about Age, infertility is very related to age, as a lady advances in age her ovarian reserves reduces.

We also have uterine malformations , that is the womb was created abnormal from birth, and also the womb can also be made abnormal by fibroids and endometriosis.

Other causes of infertility are thyroid and other hormonal imbalance.

Then men can also be a cause of infertility due to low sperm count, poor libido or erectile dysfunction.

The list can be endless, but I want to quickly go to some ways of preventing infertility.

However the truth remains that most types of infertility cannot be prevented.

Those you can prevent are

[1] Smoking , which is linked to low sperm count and poor quality egg production.

[2] Alcohol affects the fertility of both men and ladies

[3] Excercise is good, but too much stressful excercise can lead to menstrual disorder in ladies and affect sperm production in men.

[4] Been too fat or even too thin is another terrible state, that can seriously affect ovulation in ladies and sperm production in men.

[5] Eating healthy and natural food and drinks can really improve fertility

Some compulsory tests that must be done and repeated yearly or six monthly, are HSG, Folliculometry [repeated 3 monthly], Hormonal Profile, HVS and Full Blood Count, Sperm tests, Scrotal and testes scan.

IRREGULAR MENSTRUATION IS A CONSTANT ISSUE IN INFERTILITY AND PREGNANCY

This will be a wonderful weekend, so enjoy it.

Unfortunately, not many are lucky like us

The proverbial Ember month have unfortunately caught up with them.

I hope you are following the recent news, you would have learnt that over 18 people lost their lives at a ghastly accident in Alakija area of Lagos State, and another 20 people also lost their lives at the Lagos – Ibadan expressway, all just yesterday alone.

May their souls all rest in perfect peace

This is the end of the year, there is this tendency for us to be in a hurry to meet a certain target before Xmas.

You may not agree with me, but I am among those that believe that a day is just a day, no more special than any other day. That is why I work full day on Xmas, boxing day and new year.

I have been doing this for the last 16 years .

So it has made me never to be eager or be in a hurry to meet any particular target on any particular day.

In fact I never make any new year resolutions, I make monthly resolutions, and alter them frequently so as to meet my target and make success of my resolutions.

Since October, I have attended to many infertility clients, and I hear one thing

They have placed a target of getting pregnant before Xmas

In fact the statement remained like this ‘’this year must not pass me’’

I understand their sentiments, but all I ask is that it should not be attached to a certain time.

This can put pressure on the couple and affect the lady’s hormonal balance, and definitely affect her fertility badly.

Irregular Menstruation, I must tell you can be caused by anxiety, so if you are under pressure to get pregnant before December 25 , you may just be punishing your body and prolonging your conception time.

Irregular Menstruation is not a new topic,

I do not want to make this conclusion, that 90% of ladies complain of one form of irregular periods or another.

However the truth is that not all those complaining of irregular menses have them.

Irregular Menstruation or irregular cycles or periods is an abnormal variation in length of menstrual cycles in ladies.

It is unusual for a woman to experience cycle length variations of less than four days between the shortest and longest cycle lengths.

Let me make it simpler, an irregular period may be defined as one shorter than 21 days or longer than 36 days.

Irregular periods can be an unusual heavy bleeding called menorrhagia

It can also be infrequent, scanty and very light called oligomenorrhoea .

I know, many of you will say this is my case, I must confess one thing , many ladies will suffer one form of irregularity of her period because it is caused by changes in her hormone balance.

That is why I am saying, you must not do thing that can affect your mind and affect your hormones.

Things that can affect your hormones and cause irregular menstruations could be,
stress;
just starting a new menstrual cycle;
dietary problems- like eating poorly nourishing food;
too much exercise;
break in your routine-like making a sudden journey;
sexual activity- sex is good but it has a way of spiking your hormones too;
anxiety about pregnancy;
miscarriages and abortions;
even after giving birth;
illness like having a thyroid condition;
polycystic ovaries;
endometriosis;
infection;
and many other causes.

Irregular menstruation can easily revert back to normal, even without treatment .

However irregular menstruation can cause infertility, because it affect ovulation

Irregular menstruation can lead to irregular ovulation or even anovulation [no ovulation].

If you are suspecting that you are having irregular menstruation , you will need to undertake a Hormonal Profile test and Transvaginal Scan.
