I want to let you into the nature and style of my work today.

I had this lovely looking female patient, that was rushed into the clinic with sharp lower abdominal pains, nausea and vomiting. The pain was more extreme on her left lower pelvic region.

Infact you need to see her, she was in extreme lower abdominal pain that radiates to the back, side and thigh, with nausea, and vomiting. She was sweating profusely and had a fever.

We decided to give her a quick emergency ultrasound scan. During the ultrasound scan I found out that her left ovary was enlarged and tender with evidence of a torsion, while her right ovary is normal. So my conclusions were a case of Left Ovarian Torsion.

 Ovarian torsion is the twisting of the ovary due to the influence of another condition or disease. Ovarian torsion occurs infrequently only in females. It can occur in women of all ages, but most women that experience this are younger. Approximately 70-75% of cases occur in women under 30 years old. About 20% of all reported cases are in pregnant women. It is the fifth most common gynaecological emergency which can include surgical intervention.

Ovarian torsion usually arises in only one ovary at a time. They can occur in either normal or enlarged ovaries and fallopian tubes, and occasionally they develop in both.

TREATMENT: Conservative treatment of ovarian torsion includes laparoscopy  to uncoil the torsed ovary and possibly oophoropexy  to fixate the ovary which is likely to torse again. In severe cases, where blood flow is cut off to the ovary for an extended period of time, necrosis  or death of the ovary can occur. In these cases the ovary must be surgically removed.

If ovarian torsions are diagnosed and treated early, then the prognosis is favorable. However, if diagnosis is delayed, the torsions can worsen and cut off arterial blood flow into and venous blood flow out of the ovary. This results in necrosis (death) of the ovarian tissue. Delayed diagnosis can also result in problems when trying to conceive due to infertility.

By death of the ovary , I mean that it will not function again , so you can never ovulate from that ovary again.

Luckily every woman have two ovaries. However you must remember that a lady can be ovulating from one of her ovaries for a long time before the next ovary takes over, from anything like one month to several years.

So if you are surpose to ovulate from the torsed ovary, you can imagine what that means, and without ovulation you cannot be pregnant. So you are likely to suffer from infertility for a long time.

Infertility is the inability of a woman to get pregnant after one year of continous unprotected sex, or after six months if she is over 35years of age.
